Privacy Policy

The short version

I collect only the information needed to answer your questions, book your visits, send what you asked for, and care for you. I don't sell your information. I don't send health information to advertisers or social media platforms. You can ask me to stop contacting you or to delete your marketing information at any time.

If you become a patient, your medical information is protected by federal law (HIPAA) and described in our Notice of Privacy Practices. This policy covers how this website, our forms, emails and text messages handle information.

What we collect

• Information you give us. Your name, email, phone number and anything you type into a form, booking page or message, including answers to the GLP-1 Readiness Check and intake questions.
• Booking and payment details. Appointments and payments are handled by our scheduling and payment system. We don't store full card numbers on this website.
• Basic website data. Pages visited, the site that referred you, device and browser type, and general location (city level). This helps us understand which pages are useful.

How we use it

• To answer your questions and schedule your Clarity Consult, Metabolic Snapshot^™ or other visits.
• To send the guide, emails or reminders you asked for, and follow-up related to your request.
• To prepare for any visit you book and to provide your care.
• To understand how the website is used so we can improve it.
• To meet legal, safety and recordkeeping requirements.

Who we share it with

We share information only with companies that help us run the practice, and only what they need to do that job: our scheduling and medical records system, our customer and email system, payment processing, and laboratory services you are referred to. Companies that handle health information for us sign a Business Associate Agreement, which legally requires them to protect it.

We do not sell, rent or trade your personal information. We do not share your health information, Readiness Check answers or intake answers with advertising or social media platforms.

We may disclose information when the law requires it, to protect someone's safety, or with your written permission.

Text messages

If you give us your mobile number and check the box agreeing to texts, we may send appointment reminders and practice updates. Message frequency varies. Message and data rates may apply. Reply STOP to opt out at any time, or HELP for help. Consent to receive texts is never a condition of booking or care.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent are not shared with any third parties.

Texts never include detailed medical information. For anything clinical, we use secure messaging.

Emails

If you join our list or use the Readiness Check, you may receive educational emails and updates. Every email includes an unsubscribe link. Unsubscribing from marketing emails won't stop messages about appointments you've booked.

Cookies and analytics

This website uses cookies and similar tools to remember preferences and to count visits. We use analytics only on public pages, such as the home page and service pages. Analytics tools do not receive your name, contact details or health answers. Booking, intake and Readiness Check results pages are not tracked by advertising or analytics tools.

You can block or delete cookies in your browser settings. The website will still work.

GLP-1 Readiness Check

When you start the Readiness Check, we receive your first name, email, the status you select, and, if you choose to give them, your mobile number and text consent. When you open your report, we also receive your answers to the checklist. We use them to send your follow-up and to prepare for any visit you book. Your answers are stored in our secure, HIPAA-compliant system and are never shared with advertisers.

The Readiness Check is education only and is not medical advice. Do not start, stop or change any medication without talking with your prescriber. To delete your Readiness Check information, email the address below.

Your choices

• Stop marketing. Unsubscribe from any email or reply STOP to any text.
• See, correct or delete. Ask what we hold about you, ask us to correct it, or ask us to delete information used for marketing. Medical records we are legally required to keep can't be deleted, but you may request copies and corrections as described in our Notice of Privacy Practices.
• We respond to requests within 30 days and will confirm your identity first.

Keeping it safe

We use encrypted connections, access controls and systems built for health information. No system is perfect, so if we ever learn of a breach that affects your information, we will notify you as the law requires.

How long we keep it

We keep marketing information until you ask us to delete it or it is no longer needed. We keep medical records for as long as Illinois law and professional standards require.

Children

Our services and this website are for adults 18 and older. We do not knowingly collect information from children.

Changes to this policy

If we update this policy, we'll post the new version here with a new effective date. Significant changes will be highlighted on this page.
